Section 1-5701. ISSUANCE AND FEES FOR REPLACEMENT DC ONE CARDS

5701.1The Office of the Chief Technology Officer (OCTO) shall issue replacement DC One Cards.

5701.2OCTO shall charge a non-refundable fee of five ($5) for the replacement of each DC One Card that contains an electronic chip.

5701.3Cardholders may pay the replacement fee by major credit card or money order.
