Section 15-603. FILING REQUIREMENTS: TRANSFER OF OWNERSHIP AND ANNUAL REPORT

    603.1A PSP may request that its certificate be canceled.

     

    603.2A request for cancellation of the certificate shall be in writing and shall include:

     

    (a)A list of the locations of each PSP payphone(s), if any;

     

    (b)The PSP's specific plans for terminating service and removing the pay telephone(s), or its plans for transferring the PSP's registered pay telephones to another certified PSP in accordance with § 610; and

     

    (c)The date on which the PSP wishes the cancellation to become effective.

     

    603.3If a request for cancellation of a certificate meets the requirements under this section, the Commission shall within ten (10) days send written notice to the PSP that its certificate has been canceled.
