Section 16-1803. APPLICATION FOR AN ALARM AGENT LICENSE

1803.1An application for an alarm agent's license shall be made on a form prescribed by the Director and shall contain a notarized statement that the information contained in the application is true and correct.

1803.2Each application shall be accompanied by three (3) prints of a full-face photograph taken within not more than three (3) months prior to the date of the application. The prints shall be one inch by one and one-half inches (1 in. x 1 1/2 in.) in size.
