Section 16-811. APPROVAL OF CONTRACT AND RECEIPT FORMS

811.1No contractor shall use any contract form or receipt form required by this chapter unless a sample of the form has first been submitted to and approved for compliance with this chapter by the Director.

811.2If, within five (5) business days from the date of the submission of a proposed form, the Director has not approved the proposed form, the form may be used by a contractor until such time as the contractor shall have been notified by the Director, in writing, that the form is not in compliance with this chapter, and thereafter the Director shall allow the contractor a reasonable time within which to comply.
