Section 17-2902. APPLICATION FOR LICENSE

2902.1An applicant for a license to operate an animal facility is required to be a veterinarian licensed under the Act in good standing.
2902.2The owner of an existing facility shall apply for a license under the chapter within thirty (30) days the effective date of the chapter.
2902.3An applicant for initial licensure other than an applicant subject to § 2902.2 shall submit application to the Director on the prescribed form, and pay the required fee, at least sixty (60) days prior to the intended date of operation.
2902.4An applicant shall include a copy of a certificate of occupancy, and certificate of incorporation in good standing, if any, for the facility with the application.
