Section 17-5408. REINSTATEMENT OF AN EXPIRED LICENSE

    5408.1An applicant for reinstatement of a license shall:

     

    (a)Submit proof as set forth in § 5410 of having completed twenty-four (24) hours of continuing education in the applicant's current area of practice in the two (2) years immediately preceding the application date; and

     

    (b)Meet any other requirements that the Board may set forth to determine whether the license should be reinstated

     

    5408.2An applicant for reinstatement of a license shall submit the completed application and documents required by the Board and pay the required fees.

     

    5408.3The Board shall not reinstate the license of an applicant who fails to apply for reinstatement of the license within five (5) years after the license expires. The applicant shall apply, and meet the requirements in existence at that time, for licensure by examination or endorsement.
