Section 17-9331. RECORDS RETENTION

9331.1Each HHA training program shall maintain an accurate and appropriate system of record keeping.

9331.2Each HHA training program shall ensure that administrative and personnel records are protected against loss, destruction, and unauthorized review.

9331.3A record for each trainee shall be maintained by the facility and shall include the trainee’s evaluation and health information.

9331.4Each instructor’s personnel records shall be maintained by the facility and shall include application data, qualifications, and position description.

9331.5All records shall be maintained by the HHA training program for a minimum of five (5) years.
