Section 18-419. OFFICIALLY ALTERED REGISTRATION CARD

419.1Upon delivery to the Director of a registration card which contains an error which, in the judgment of the Director, does not necessitate the issuance of another card, the Director shall make the necessary correction and place the words “OFFICIALLY ALTERED,” the date, and the Director’s signature on the card and deliver it to the registrant or the registrant’s agent.
419.2A registrant who changes his or her name by marriage shall, within thirty (30) days after the marriage, deliver to the Director his or her registration card accompanied by a written statement on a form provided by the Director and containing the information required by the Director.
419.3A registrant who changes his or her name other than by marriage, shall within five (5) business days after the change, deliver to the Director, his or her registration card accompanied by a written statement on a form provided by the Director and containing the information required by the Director.
419.4When satisfied that a registrant is entitled to a change in the name appearing on his or her card, the Director shall make the change and place the words “OFFICIALLY ALTERED,” the date, and the Director’s signature on the card and deliver it to the registrant or the registrant’s agent.
