Section 21-711. APPLICATION FOR LICENSE

    711.1Applications for solid waste collector's and collection vehicle licenses shall be submitted on approved forms to the Director.

     

    711.2The fees for solid waste collector's and collection vehicle licenses shall be submitted with the applications to the Director.

     

    711.3Each application shall include the following:

     

    (a)The name of the person to be licensed;

     

    (b)If the applicant is other than an individual, the name and title of the applicant; and

     

    (c)The address and telephone number of the location to which the license will apply.

     

    711.4Each application shall also contain the following information concerning each vehicle to be licensed:

     

    (a)Type, make, and year of manufacture;

     

    (b)Tare weight in pounds;

     

    (c)Capacity in cubic yards;

     

    (d)-Jurisdiction of registration; and

     

    (e)Motor vehicle license number.
