Section 22-B3506. PROGRAM STATEMENT

3506.1Each GHMRP shall have a written statement of its philosophy and programmatic goals which shall include, at a minimum, the following:

(a)The number and types of residents to be served;

(b)The characteristics of the residents to be served;

(c)A list of the services available to residents including local community resources;

(d)The sources of referral of residents;

(e)The GHMRP’s relationship with parents, advocates and legal guardians; and

(f)The name(s) of the licensee, operator and contact person.

3506.2The program statement shall be made available for distribution to each person who requests it.

3506.3The program statement shall be revised, as needed.
