Section 22-B4505. ANNUAL DISCHARGE DATA REPORTS

    4505.1Each hospital, unless specifically exempted by SHPDA, shall provide annually, in a form, format, and medium designated by SHPDA, a hospital discharge data set including the following information for each patient:

     

    (a) Age;

     

    (b) Sex;

     

    (c) Race;

     

    (d)Major hospital service;

     

    (e)Disposition;

     

    (f)Patient residence;

     

    (g)Admission date;

     

    (h)Discharge date;

     

    (i)Expected source of payment;

     

    (j)Principal diagnosis and other diagnoses;

     

    (k)Principal procedure and other procedures;

     

    (l)Hospital identification;

     

    (m)Patient sequence number (assigned in a way unrelated to the medical record number);

     

    (n)Birth weight in grams (neonates only);

     

    (o)Financial data;

     

    (p)Ward; and

     

    (q)Other data that SHPDA may require.

     

    4505.2Hospital discharge data shall be submitted not later than one-hundred and twenty (120) days following the end of the year being reported upon.
