Section 22-C804. NOTIFICATION OF END OF QUALIFYING MEDICAL CONDITION OR TREATMENT

804.1A physician shall notify the Department in writing within fourteen (14) calendar days after advising a qualifying patient that he or she no longer suffers from a qualifying medical condition or treatment.
