Section 25-F804. ISSUANCE OF LICENSE - NOTICE OF OPENING, DISCONTINUANCE OF OPERATION, AND POSTING

    804.1A tanning facility shall provide notice to the Department of its intent to operate at least thirty (30) calendar days before beginning operations.

     

    804.2A tanning facility shall provide notice to the Department of its intent to shut down permanently or temporarily at least thirty (30) calendar days before discontinuing operations.

     

    804.3If a tanning facility is closed for more than a thirty (30) day period, the tanning facility’s license and certificate of occupancy shall be returned to the Department and the owner shall be required to submit a new application as specified in Section 801.5 for the issuance of a new license prior to reopening.

     

    804.4A current inspection report, and all valid licenses, Certificate of Occupancy and “Warning Statements” required in Sections 302.3, 800.1, 800.2 and 800.3, and shall be conspicuously posted in the reception area next to the tanning facility’s license.
