Section 29-2510. FAMILY ASSESSMENT

    2510.1A family that receives a shelter or supportive housing placement shall be referred for an assessment with an appropriately trained, qualified, and supervised assessment specialist.  The purpose of the assessment is to:

     

    (a)   Assess the family’s full range of needs, including housing, medical, behavioral, economic, educational, and employment needs;

     

    (b)   Develop an initial Service Plan, in consultation with the client and the Client Advocate, if applicable; and

     

    (c)   Make any necessary referrals based on the family’s immediate needs and priority determination.

     

    2510.2The family assessment shall be primarily conducted by a licensed social worker or other qualified, certified or licensed, professional.  Other professionals, including psychologists, psychiatrists, and other professionals relevant to a client’s needs, may also participate in the assessment as needed. 

     

    2510.3 The family shall be re-assessed at specified intervals as determined by the family’s Service Plan.
