Section 29-2555. ADMINISTRATIVE REVIEW RECORD

    2555.1The Department shall maintain a record for each administrative review offered or held.  Each administrative review record shall include:

     

    (a)Documentation of the request for a fair hearing;

     (b)Documentation of the notice of the administrative review;

     (c)Evidence considered at the administrative review, if held;

     

    (d) All status reports issued to the Office of Administrative Hearings; and

     

    (e) All administrative review decisions issued.
