Section 29-4201. ELIGIBILITY

4201.1As a condition of waiver participation in the home- and community-based waiver services program, each recipient or applicant must receive case management services for each service(s) provided.

4201.2No recipient shall receive case management services unless the recipient or an authorized representative of the recipient signs the "Beneficiary Freedom of Choice Form" provided by the recipient's case manager to elect to receive home and community-based waiver services.
