Section 31-329. CONTENTS OF DOCUMENTS FILED IN PROCEEDINGS

    329.1If there is no rule or order of the Commission, Panel, or Office which prescribes the content of a formal application, petition, complaint, motion, or other authorized or required document, the document shall contain a proper identification of the parties concerned, the docket number, if assigned, and a concise but complete statement of the facts relied upon and the relief sought.

     

    329.2Every document shall be signed by the party filing it, or by a duly authorized officer of a business organization or the attorney of record of that party.

     

    329.3The signature of the person signing the document constitutes a certification that he or she has read the document; that to the best of his or her knowledge, information, and belief every statement contained in the document is true, and no such statements are misleading; and that the document is not interposed for delay.

     

    329.4The initial document filed by any person shall state on the first page the name and post office address of the person or persons who may be served with any documents filed in the proceeding.

     

    329.5A document which is filed in violation of this section or in violation of any other requirements imposed by this chapter shall not be accepted for filing by the Office and shall not be physically incorporated in the docket of the proceeding. The sender of the document and all persons who have been served therewith shall be notified of the action of the Office.
