3001.1Each report, claim, or application required by the Act or this chapter shall be made on forms approved by the Deputy Chief Financial Officer of the Office of Tax and Revenue or in a manner prescribed or approved by the Deputy Chief Financial Officer.

3001.2Approved forms may be obtained from Room 480, 441 4th Street, N.W., Washington, D.C. 20001.
