Notice of Public Hearing

 

Pursuant to 22 DCMR B § 4302 the District of Columbia State Health Planning and Development Agency (“SHPDA”) will hold a hearing on the following (2) certificate of need applications:

 

1)      RAI Care Centers of Southeast DC, LLC for the Establishment of a 24-Station Dialysis Facility and Home Hemodialysis and Peritoneal Dialysis Training Programs at 1918 14^th Street, S.E.- Certificate of Need Registration No. 12-8-4

 

2)      Bio-Medical Applications of the District of Columbia, Inc. for the Establishment of a 16-Station Dialysis Facility and Home Hemodialysis and Peritoneal Dialysis Training Programs at 745 Howard Road, S.E.- Certificate of Need Registration No. 11-5-16

 

The Hearing will be held on Thursday, May 10, 2012 at 11:00 a.m., at 899 North Capitol Street, N.E., 4^th Floor, Room 407, Washington, D.C. 20002.

 

Testimony from affected persons will be received at the hearing. Comments may be submitted in writing before the hearing, or they may be presented at the hearing orally or in writing. Written Statements may also be submitted to the SHPDA, 899 North Capitol St, N.E., Second Floor, Washington, D.C. 20002, until 4:45 p.m. on Thursday May 17, 2012 before the record closes. The referenced applications are available at the SHPDA for review.

 

Persons who wish to testify should contact the SHPDA on [phone] before 4:45 p.m. by Wednesday, May 9, 2012. Each member of the public who wishes to testify will be allowed a maximum of five (5) minutes.
