STATE HEALTH PLANNING AND DEVELOPMENT AGENCY

 

NOTICE OF PUBLIC HEARING

 

 

 

 

Pursuant to 22 DCMR, Section B4302.1, the District of Columbia State Health Planning and Development Agency ("SHPDA") will hold a public hearing on an application by Aquila Recovery, Chartered for the establishment of outpatient substance abuse treatment services at 4455 Connecticut Avenue, N.W., Certificate of Need Registration No. 11-3-2.  The hearing will be held on Wednesday, August 10, 2011 at 11:00 a.m., at 899 North Capitol Street, N.E., 4^th Floor, Room 407, Washington, D.C.  20002.

 

Persons who wish to testify should contact the SHPDA on [phone] before 4:45 p.m., by Tuesday, August 9, 2011.  Each member of the public who wishes to testify will be allowed a maximum of five (5) minutes.  Written statements may be submitted to:
