NOTICE OF INFORMATION HEARING

 

 

 

 

Pursuant to D.C. Official Code § 44-406(b)(4), the District of Columbia State Health Planning and Development Agency ("SHPDA") will hold an information hearing on the application by Transitions Healthcare Capitol City, LLC and 2425 25^th Street, S.E., LLC for the Acquisition of Washington Nursing Facility - Certificate of Need Registration No. 14-8-2.  The hearing will be held on Friday, July 25, 2014, at 11:00 a.m., at 899 North Capitol Street, N.E., 4^th Floor, Room 407, Washington, D.C.  20002.

 

The hearing shall include a presentation by the Applicant, describing its plans and addressing the certifications provided pursuant to D.C. Official Code § 44-406(b)(1) and an opportunity for affected persons to testify.  Persons who wish to testify should contact the SHPDA on [phone] before 4:45 p.m., by Thursday, July 24, 2014.  Each member of the public who wishes to testify will be allowed a maximum of five (5) minutes.  Written statements may be submitted to:
