NOTICE OF INFORMATION HEARING

 

 

 

 

Pursuant to D.C. Official Code § 44-406 (b) (4), the District of Columbia State Health Planning and Development Agency ("SHPDA") will hold an information hearing on the application by Community of Hope for the acquisition of Family Health and Birth Center, Certificate of Need Registration No. 11-5-2.  The hearing will be held on Monday, September 12, 2011 at 11:00 a.m., at 899 North Capitol Street, N.E., 4^th Floor, Room 407, Washington, D.C.  20002.

 

Persons who wish to testify should contact the SHPDA on [phone] before 4:45 p.m., by Friday, September 9, 2011.  Each member of the public who wishes to testify will be allowed a maximum of five (5) minutes.  Written statements may be submitted to:
