Section 26-A1402. FILING OF STATEMENTS

1402.1Initial statements of beneficial ownership of equity securities required by Section 3(a) of the Act shall in all respects comply with the instructions set forth in Schedule A of Appendix 14-1 and shall be filed in a form as the Commissioner may prescribe.

1402.2Statements of changes in a beneficial ownership required by Section 3(a) of the Act shall in all respects comply with the instructions set forth in Schedule B of Appendix 14-2 and shall be filed in a form the Commissioner may prescribe.
