Section 29-147. FILING OF DOCUMENTS

    147.1No particular form shall be prescribed for documents which may be filed in proceedings under these rules.

     

    147.2Each request for a hearing, or other document which may be filed in proceedings under these rules, shall meet the following requirements:

     

    (a)Be clearly legible;

     

    (b)Contain a clear, concise statement of the facts and the relief sought; and

     

    (c)Be signed by the party involved or the party’s representative.

     

    147.3Each document, including the request for a hearing, shall be filed with the Office of Administrative Hearings.

     

    147.4When a document is filed, the filer shall serve a copy of the document on each party in the action. In any case in which an applicant or client has designated a representative, the representative shall also be served a copy of the document.

     

    147.5Unless otherwise ordered by the hearing examiner, filing of documents may be accomplished by first class mail.

     

    147.6Filing shall be considered complete upon receipt.
