    STATE HEALTH PLANNING AND DEVELOPMENT AGENCY

     

    NOTICE OF INFORMATION HEARINGS

     

     

    Pursuant to D.C. Official Code § 44-406(b)(4), the District of Columbia State Health Planning and Development Agency ("SHPDA") will hold two (2) information hearings on the following two (2) certificate of need applications by:

     

    Howard University Dialysis Center, LLC for the Acquisition of the Howard University Hospital Dialysis Center Located at 2041 Georgia Avenue, N.W. - Certificate of Need Registration No. 11-1-2

     

    and

     

    Total Renal Care, Inc. for the Acquisition of the Assets of International Dialysis Center, LLC Located at 1730 Hamlin Street, N.W. – Certificate of Need Registration No. 11-5-16.

     

    The hearings will be held consecutively on Wednesday, February 1, 2012, beginning at 11:00 a.m., at 899 North Capitol Street, N.E., 4th Floor, Room 407, Washington, D.C.  20002.

     

    Each hearing shall include a presentation by the Proposed Owner, describing its plans and addressing the certifications provided pursuant to D.C. Official Code § 44-406(b)(1) and an opportunity for affected persons to testify.  Persons who wish to testify should contact the SHPDA on (202) 442-5875 before 4:45 p.m., by Tuesday, January 31, 2012.  Each member of the public who wishes to testify will be allowed a maximum of five (5) minutes.  Written statements may be submitted to:
